CHS Parents,

CHS 2014 – 2015 is rapidly drawing to a close.

As we strive to continue to improve CHS we need your help.

Please fill out the online survey (see link below) and provide your feedback. If we are doing something well, let us know. If you see areas of improvements, definitely let us know.

When you fill out the survey, you will be provided a confirmation number.
The survey confirmation will count towards your child’s homework. Please fill out a survey for each child attending CHS. The confirmation number is system generated at the beginning of the survey. So do a screen shot or cut-and-paste of the confirmation number provided at the beginning of the survey. Send the confirmation number to your child’s teacher via email.

Children in C4/CSL4 and higher grades are welcomed to fill out the online survey. Parents of children in C3/CSL3 and lower grades should fill out the online survey.

Please fill out the online survey and send a screenshot or cut-and-paste of the confirmation number to your child’s teacher for Homework credit by Saturday, April 18, 2015.
