Spring is the time for CHS’ annual Parents Opinion Survey. As in prior two years, the survey will be conducted online.

Your feedback is important to help the school improve. To ensure participation of this survey by all students, CHS teachers have decided that this survey will be counted as one homework grade. Since there is no name or e-mail associated with each survey, it is therefore required that you supply the confirmation number upon completion of the survey. You can find the confirmation number after you select the language class. Please jot down the confirmation number as shown and provide it to the teacher to claim credit for the homework.

Please use one form per student. Language class students can enter one form for both language class & Cultural / Special Interest class that they attend. Adult or Special Interest class only students can also use this form to provide feedback. Survey results are collected anonymously (without your e-mail address or name). Please do not submit any form twice for the same student. Summary of data is tabulated and shared with each teacher.

Please complete the survey by Saturday April 14th, 2012.
